Advance care planning for your patients can reap benefits

Posted on: 12/20/17

Did you know that CMS is now reimbursing on average $86 per session every time you discuss advance care planning with your Medicare patients?  Every patient needs an advance care plan. Advance care planning should be a regular topic you cover with all patients, whether they are currently seriously ill or not. Accidents can happen at any time, regardless of age and health status. Even for those who have made an advance care plan, over the course of their life and changing circumstances their wishes about end-of-life care may evolve.
