Description:

The OIG and CMS claims processing contractors have been conducting a significant number of investigations related to electronic medical record (EMR) documentation and evaluation & management (E/M) coding. A substantial number of overpayments have been identified based on upcoding directly attributable to inaccurate and cloned documentation. This session walks through a case study which resulted in an OIG self-disclosure related to these and other issues associated with the use of EMRs.
