KEPRO can partner with your hospital to offer free Patient Navigation Program

The Centers for Medicare & Medicaid Services (CMS) has launched a program for people with Medicare that could be useful for your hospital. It’s called the Patient Navigation Program and is administered by Beneficiary and Family Centered Care Quality Improvement Organizations (BFCC-QIOs). KEPRO, the BFCC-QIO for our state, has established a Patient Navigation program as part of its Person and Family Engagement (PFE) initiative, to coordinate with the goals of CMS.
KEPRO would like to partner with hospitals to offer this free program to Medicare Fee-for-Service patients who are being discharged. This program offers follow-up to patients as they transition from the hospital to home. Providing patients with this type of care coordination can reduce the risk of hospital readmissions, which benefits hospitals by reducing their readmission rates, and also helps patients with a smoother transition. Some of the services that Patient Navigation provides include:
- Assisting with finding transportation options to the doctor’s office for an appointment.
- Helping to identify community resources that a patient may need after discharge.
- Helping patients and families understand their diagnosis and treatment plan.
