Recruiting organizations to be featured in CDC’s Million Hearts Tobacco Change Package

OHA’s Health Improvement Initiatives team, in partnership with the American Lung Association, is supporting an update of the Million Hearts tobacco change package

The Tobacco Cessation Change Package (TCCP) is a quality improvement tool created by the Centers for Disease Control and Prevention (CDC). This toolkit provides resources for health care professionals to increase the reach and effectiveness of tobacco cessation interventions and to incorporate these interventions into the clinical workflow.

OHA has been asked to provide resources that would be added to the updated version of the TCCP. We want to invite our hospital partners to contribute resources to this toolkit and add to its success and impact. This is a huge opportunity to celebrate the tobacco cessation efforts of your team that will be seen and used by health systems throughout the nation. We would love to include any testimonials that celebrate your team’s achievements and share their stories to inspire other hospitals. If you have any best practice documents, guidelines, case studies, and/or applicable templates that can aid in clinical tobacco treatment systems change, we would love to include that as well.
