The OHA Excellence in Clinical Quality and Safety Awards recognize exceptional efforts to improve clinical quality and patient safety in Oklahoma hospitals. These awards represent the highest level of professional acknowledgement from the OHA board and the OHA council on quality and safety.
Eligibility Requirements
All OHA member hospitals are eligible to apply for the OHA Excellence in Clinical Quality and Safety Awards, describing a project-based clinical topic area implemented within the timeframe of calendar year 2023 to the present.
The OHA Excellence in Clinical Quality and Safety Awards will be given to one member hospital in each of the following categories (based upon licensed beds):
- < 25 Beds (Including Critical Access)
- 26-100 Beds
- 101-300 Beds
- > 300 Beds
- Specialty Hospital (This may mental health, surgical, free-standing ED and other)
Submission Requirements - Deadline Wednesday, July 1, 2026, by 5 p.m.
1. Complete the OHA Excellence in Clinical Quality and Safety Awards
application.
2. Applicants must provide the following for successful submission of the application:
- CEO/CNO Affidavit – completed.
- Visual Representation – include at least six months of data reflecting improvement (graph, table, run chart, etc.)
- Team Photo - this must be a high-resolution photo, including a list of each member’s name and title from left to right.
- Facility Logo - this must be a high-resolution/original file with transparent background. Screenshots of logo will not be accepted.
Late submissions will not be considered.
Selection and Recognition of Awards
One award recipient will be selected for each category based upon bed size, as described above. Recipients of the OHA Excellence in Clinical Quality and Safety Awards will receive prior notification and will be asked to present their project during the 2026 OHA Connect conference on Sept. 30, 2026. Award winners and their organization will be recognized during the 2026 OHA Connect Awards Dinner on the evening of Sept. 30, 2026. Award recipients will receive the following award package.
- Free registration to attend OHA Connect for each project owner (Includes two free OHA Connect Awards dinner tickets for the project leader and a guest)
- A storyboard poster to display at OHA Connect
- Inclusion of storyboard poster in an awards compendium, published on the OHA website
- An award plaque
- Professional photo of hospital staff/leadership with OHA president
- Public recognition via media release with support from OHA
